  1. All the fields are required to be filled. Please type n/a or N/A if the fields are irrelevant to you.
  2. Certified copies of your scanned/electronic documents must be submitted together with your online application:

    a. Identity Card (I/C)
    b. SPM result or equivalent; if applicant is waiting for his/her results, please submit the school’s forecast results or trial examination results.
    c. STPM result or any Diploma / Certificate or equivalent (if applicable)
    d. School Leaving Certificate / Testimonial.

    Please ZIP all your documents as 1 single file as only .zip & .ZIP file extensions are allowed. The size limit per attachment is 5 MB. If your file is larger than that, please make arrangement to send the documents via fax or mail.

  3. Form with missing information or without attachment will not be accepted.
  4. If you do not have scanned/electronic documents to attach, alternatively, you can download the Application Form & mail it to us with your certified paper documents.
